Self-Assessment — Complex PTSD

The Complex
PTSD Scorecard

Complex PTSD develops not from a single event, but from prolonged or repeated experiences that kept the nervous system in survival mode. Many people carry it for years without ever having a name for what they are experiencing.

This scorecard covers eleven areas that C-PTSD touches. Answer each question as honestly as you can — based on how things genuinely are for you right now, not how you would like them to be. There are no right or wrong answers. There is only the true one.

To understand more about Complex PTSD before you begin — what it is, what it feels like from the inside, and why it develops — read our full guide.

How the grading works: each question uses an A to F scale. For most questions, A means this pattern is not present in your life and F means it is a constant feature. When you select a grade, a short description will appear beneath the buttons. Read it before moving on to make sure your answer genuinely fits.

Take your time. Some of these questions may bring things to the surface that you have not looked at directly before. That is okay. You can pause at any point. The scorecard will wait.

This is a self-reflection tool, not a clinical assessment. It does not provide a diagnosis. If you recognise yourself strongly in these questions, we encourage you to book a free 30-minute call with us after completing it.
